Client Rights & REcord Access

Access to Health Records

Clients have the right to request access to their health records in accordance with state and federal law, including HIPAA.

To request records, please submit a written request via email, your client portal, or by contacting me directly. Requests are processed within the timeframes required by law. Reasonable fees may apply as permitted.

Licensing & Complaints

I am licensed to provide psychotherapy services in the following states. Each state maintains a regulatory board to oversee licensure and consumer complaints:

Clients may file a complaint with the appropriate licensing board in the state where services were provided. Complaints related to privacy or access to health information may also be submitted in accordance with applicable laws.

Clients are encouraged to discuss concerns directly with me whenever possible; however, this does not limit the right to file a formal complaint.
